You have to edit the Fallout4Prefs.ini in the Documents>My Games>Fallout4 folder and set the values of iSize H=1080 iSize W=2560 (for example). I run it in Fullscreen mode (taking up the entire screen) by using bMaximizeWindow=0 bBorderless=0 bFull Screen=1 in there as well (this is all under the DISPLAY category).
Im playing on a 21:9 ultrawide at 3440x1440p, and realized that Fallout 4 (and new vegas) aren't optimized for ultrawide resolutions, but I cant seem to get them to perform well. I downloaded a mod for Fallout 4 to scale the game properly to ultrawide and changed my resolution in the game files but it runs at like 30-40fps with dips constantly.

It's a massive pain in the ass. Yup. Flawless Widescreen is also incredible. I couldn’t get Fallout 4 to run smoothly (on a high end machine) and just gave up. I did get it to run in ultrawide mode, but it was like the frame rate was cut in half. So I needed 100+ FPS for it to feel like 60, and 60 felt more like 30.With Skyrim at least I can just use mods from the workshop and do fine.**.
